WHAT IS IV CHAINING?
IV Chaining, in the context of the Pokemon series, is encountering and defeating the same Pokemon over and over again to generate an encounter with what are called "perfect" IVs, a value of 31. Done correctly, you can be granted a Pokemon with a guaranteed 4 perfect IVs. Potentially more, if RNG feels like being nice.

WHAT IS DEXNAV?

DexNav, in the context of the Pokemon games, is a function added to Omega Ruby and Alpha Sapphire. The more you encounter a Pokemon, the more likely things will happen to manipulate the next spawn. Any combination of things can happen, including perfect IVs, Hidden Abilities, Egg Moves in the first move slot, and shiny rerolls.

HOW IS IT DONE?

The way this addon works is, whenever you defeat or catch a Pokemon, the counter will check if it's part of the chain. If yes, it increments the counter. If not, the counter resets and tracks the new Pokemon instead. Upon spawning of a chained Pokemon, the addon will then override random IVs to make them perfect. 5 encounters gives one guaranteed perfect IV. 10 gives 2. 20 gives 3. 30, the cap, gives four guaranteed IVs.
To check your IV chaining progress, you can use the command /ivchain, or craft a DexNav item

This mod is serverside only. This means that it can be installed on a server without the client requiring it to be installed. (However, if you want chaining on a SSP world, that works, too.)
