You will start up the world in Survival mode at the entrance to the maze (the feet). In order to get creative mode, you must make it to the end of the maze and stand on the pressure plate (the cat's nose).
Just unzip and extract the file, then put it in your Minecraft saves folder, start up Minecraft in version 1.12, and open up the world in singleplayer!
