The difference between the both hearts is like Paid one has more hearts and they are craftable and there are 9 hearts in total.

This hearts are craftable using there own custom crafting recipes. Which can be seen in Crafting Table.

Well you can obtain Free Custom Hearts in Creative mode only, but in Premium (support) version you can craft them and they have custom powers!
